SUMMARY OF ACTIVITIES, FOR THE REGION COMMANDER     

CHAPTER INPUT FORM and SAMPLE FORM 

1. INPUT FORM: Click on the word "Input" above. You may then enter your chapter's activities directly on the form. When you click on the submit button, the data will be sent by e-mail to the webmaster, who will post the form under your chapter designation shown at the top of this page. 

2. SAMPLE FORM: Click on the word "Sample" above and download a copy of that form to your own printer. You may then fill it out manually and mail it to the Region Commander who, at his discretion, may have it published on the website as mentioned in paragraph 1. above.

3. Either form will serve to summarize your chapter activities for the Region Commander and will also become your application to the Region Awards Chairmen, who will adapt it to qualify for the Small (CAPT Leo Lazo), Satterlee (LT Judy Moreau) and Dee (LTC Michael Jarvis) awards to be presented at the annual Region Convention. See "Awards" under "Records" for these results.
